Kisah sekuel keempat ini berlangsung di tengah-tengah rekonstruksi pasca perang pada musim dingin 2028. Ketegangan meningkat di New Port City demonstrasi diadakan mengenai kepentingan kartel asing. Hal ini menyebabkan insiden penembakan yang melibatkan polisi anti huru hara. Semuanya dimulai dengan infeksi cyberbrain yang dilakukan oleh teroris “Fire Starter.” Sebuah unit ofensif independen yang dipimpin oleh Makoto Kusanagi yang memimpin Ghost dan bertujuan untuk keadilan mereka sendiri. Dikarenakan kejadian itu, “tin girls” Emma dan “scarecrow man” Burinda Junior. Seperti Kusanagi, orang yang berkaitan dengan insiden tersebut semakin dekat dengan apa yang mereka mencari.
